Passports

Attention Passport Customers:  Please fill out your passport form before arriving for your appointment.  Forms can be found at travel.state.gov to be downloaded and printed or stop by our office today to pick up a paper form.  All passport forms are free please ensure you are accessing a .gov website to access the FREE forms.  Please call our office prior to your appointment at 503-434-7518 if you need assistance.


Passport Processing Times

  • Routine Service 4 - 6 weeks
  • Expedited Service 2 - 3 weeks (additional fee of $60.00 is required)

PLEASE NOTE:  Appointments are required in order to apply for passports.  Appointments are not needed for passport renewal photos.
Passport applications must be completed prior to the time of the appointment, or your appointment will be rescheduled.  In consideration of other appointments scheduled, we ask that you be on time for your appointment, or we may have to reschedule your appointment to another day.  Appointments are available Monday through Friday 8:30 a.m. - 3:30 p.m.

Passport Forms

To apply for a U.S. passport for the first time or to renew a passport by mail, applications can be downloaded from the Passport Agency's website at www.travel.state.gov or are available at this office.  If you would like to print a form, please see the links below in the supporting documents section.  Forms should be filled out completely using black ink.  Do not use white out to correct mistakes.  

Basic Requirements

All applicants must appear in person.  Please bring your completed passport application (DS-11) to your appointment.  Do not sign the application before your appointment each signature must be witnessed by a passport acceptance agent.

Evidence of Citizenship

Original documents must be submitted photocopies will not be accepted. Your original documents will be returned to you by the U.S. Department of State.  You will need one of the following:

  • A previously issued U.S. Passport (even if it is expired)
  • A state-certified Birth Certificate issued by the city, county or state where you were born.  It must have a filing date and official seal from the governing agency. (Hospital records are not acceptable). No photocopies or California Abstracts or Texas Abstracts will be accepted.
  • Consular report of birth abroad
  • Certificate of Naturalization

Photo Identification (Children under 18 can use parent's ID)

Identification must bear both the signature and the photo of the applicant.  Identification must not be expired.  The most common forms of ID are:

Passport Photos

  • Picture taken within the last 6 months. (two identical photos are required)
  • Photo size: 2 inches by 2 inches taken against a plain white or off-white background.  No uniforms or items resembling uniforms can be worn.  No camouflage, glasses, hats or bows that obscure the hair or hairline.  
  • Please review your photo against the U.S. Department of State's requirements before coming to our office to process your passport application.
  • Photos are available at the Clerk's office for a fee of $18.00.

Fees

  • Fees paid to the U.S. Department of State must be in the form of a check or money order and must be one per applicant (i.e. a family of 4 would issue 4 separate checks to the U.S. Department of State).

  • Processing fees paid to the Yamhill County Clerk may be in the form of a check, money order, credit/debit card or cash.
